JPA SCHOLARS

Salam buat rakan-rakan JPA 06. Berikut ada sedikit info daripada En Jazman mengenai urusan klaim Skripsi, Kitas, dan elaun masuk Koas:

1. Skripsi:

a. Dokumen yg diperlukan untuk klaim:

i. Skripsi yg dh jilid (kuning) berserta tanda tangan pembimbing

ii. Borang tuntutan yg telah diisi

· Borang boleh download dari blog JPA En Jazman

· Sila isi nilai tuntutan RM 605 pada borang tersebut.

b. Borang dan skripsi dihantar sekali ke JPA Jakarta

c. Untuk pembayaran klaim akan dilakukan berasingan

i. Ini bermaksud wang klaim tidak akan masuk bersama dengan elaun bulanan

ii. Nilai klaim adalah RM 605

2. Kitas:

a. Dokumen yg diperlukan untuk klaim:

i. Resit pembuatan Kitas

ii. Borang tuntutan yg telah diisi

· Borang bleh download dari blog JPA En Jazman

iii. Fotokopi kitas yg telah siap

b. Borang dan resit dihantar sekali ke JPA Jakarta

c. Untuk pembayaran klaim akan dilakukan berasingan

i. Ini bermaksud wang klaim tidak akan masuk bersama dengan elaun bulanan

3. Elaun masuk Koas:

a. Elaun masuk Koas akan diberikan kepada mahasiswa yang telah masuk Koas

b. Elaun diberikan SEKALI setelah masuk Koas

c. Terdapat beberapa syarat dan dokumen yg diperlukan untuk klaim elaun Koas

d. Urusan Klaim akan diuruskan setelah masuk Koas:

i. Setelah perjumpaan dengan En Jazman setelah masuk Koas nanti

P/s: Bagi yg buat Kitas dengan Padma dan belum menyerahkan passport, sila lakukan secepat yg mungkin.

KOAS PRELUDE

these are things u need for koas.please proceed.

1. Baju Jaga (kelabu biru)

• 1 pasang = Rp 110,000
• perempuan boleh request for long sleeve at the same price.laki pon leh gk kalo nak.
  

2. Lab Coat

• i know the colors arent exactly like the real cloth.but the point is they are all a different shades of white.nnt korg pilih la sniri.for the coat,color mcm differ a bit xpe kot.but yg penting the cutting/format/design.sbb for koas kite nye coat have to be labuh and lengan panjang

oke.for item 1 and 2,batch committee dh tlg carikan tailor,hafiz actually yg cari.so,sesi pengukuran akan diadakan pada 29 Jun 2010 at Griya Amira. Untuk para cewek, its from 8.00 am to 11.30 am. while for cowok is from 11.30am til 1.00pm.please be punctual and bawa duit yang cukup untuk bayar on dat day itself k. baju akan siap dalam +/- 1 bulan macam tu, so if u guys balik after judicium 2, sempat la ambil baju dulu before balik.btw, u are advised to have 2 of each item 1 and 2 tapi ikut suka hati la nk banyak mana kan.and kalo terasa nk jimat sebab kalo balik buat kat malaysia x guna duit sniri (ayat sopan for guna duit FaMa), korang order la 1 of each, then masa balik malaysia buat la.sbb kalo buat sendiri takut lain plak jadinya.that is just a suggestion.think and decide k.

3. Alat-alatan

• 1 termometer digital
• 1 spygmomanometer + 1 cuff untuk pediatric (preferably yg meter not mercury)
  
• 1 pen light
• 1 stetoskop adult
• 1 stetoskop pediatric
• 1 patellar hammer (bhgn saraf)
  
• 1 box of glove and 1 box of mask
• 1 measuring tape (bhgn anak)
• 1 pembaris (utk ukur JVP kat bhgn IPD)
• 1 sling bag that can fit all the above equipment as needed

alat2 ni korg leh beli sniri kat malaysia or kat sini.sini lagi murah.for dekat sini.refer map below:

pesanan senior: boleh je nk pinjam kwn punya kalo x overlap department,tapi lebih baik beli sendiri, drpd nnt pinjam,terhilangkan org punya dll.

DINNER BATCH UPDATED

the batch committee have decided that we should have a dinner marking our epilogue in jatinangor.provided below are the info:

• DATE : depending on our judicium.but for now, its either on july 1st or july 5th.a poll will be conducted later to decide the date.dates are still subject to changes (since fk still has not release the judicium date).

• VENUE : Maja House, jl. sersan bajuri 72, bandung.its on the way to lembang.transport will be provided.the website isn't ready yet,but click here for review from www.freemagz.com

• TIME : 7pm - 10 pm
  

• THEME : for now,we have only decided on the color: blue, purple and green.
  

• PRICE : Rp 100,000/person. buffet style. transport included.fees are to be collected by respective wakil kosan and submitted to our beloved treasurer,mr. hafiz by june 20th.
