Salam buat rakan-rakan JPA 06. Berikut ada sedikit info daripada En Jazman mengenai urusan klaim Skripsi, Kitas, dan elaun masuk Koas:

1. Skripsi:

a. Dokumen yg diperlukan untuk klaim:

i. Skripsi yg dh jilid (kuning) berserta tanda tangan pembimbing

ii. Borang tuntutan yg telah diisi

· Borang boleh download dari blog JPA En Jazman

· Sila isi nilai tuntutan RM 605 pada borang tersebut.

b. Borang dan skripsi dihantar sekali ke JPA Jakarta

c. Untuk pembayaran klaim akan dilakukan berasingan

i. Ini bermaksud wang klaim tidak akan masuk bersama dengan elaun bulanan

ii. Nilai klaim adalah RM 605

2. Kitas:

a. Dokumen yg diperlukan untuk klaim:

i. Resit pembuatan Kitas

ii. Borang tuntutan yg telah diisi

· Borang bleh download dari blog JPA En Jazman

iii. Fotokopi kitas yg telah siap

b. Borang dan resit dihantar sekali ke JPA Jakarta

c. Untuk pembayaran klaim akan dilakukan berasingan

i. Ini bermaksud wang klaim tidak akan masuk bersama dengan elaun bulanan

3. Elaun masuk Koas:

a. Elaun masuk Koas akan diberikan kepada mahasiswa yang telah masuk Koas

b. Elaun diberikan SEKALI setelah masuk Koas

c. Terdapat beberapa syarat dan dokumen yg diperlukan untuk klaim elaun Koas

d. Urusan Klaim akan diuruskan setelah masuk Koas:

i. Setelah perjumpaan dengan En Jazman setelah masuk Koas nanti

P/s: Bagi yg buat Kitas dengan Padma dan belum menyerahkan passport, sila lakukan secepat yg mungkin.

OF MINOR THESIS SEMINAR

I. DRAFT SUBMISSION

1. Minor thesis’ draft for 2010 seminar should be submitted in May 10-12, 2010

2. Submitted draft should be:

i. Covered with transparent plastic and bound with black tape.

ii. 6 copies

iii. Submitted through SBP

II. PRE SEMINAR PREPARATION

1. Check your seminar date and session (D-7)

2. Check the name and department of your supervisors AND examiners

3. Collect examination file folders (including draft of YOUR MINOR THESIS) from SBP

4. Deliver the folders to each of the supervisors and examiners

5. Submit the signed delivery order form at least on Friday morning (4 June, 2010) to SBP

III. RULES OF MINOR THESIS SEMINAR

1. Check your friends that share the same room with you

2. Prepare your presentation files together (ONE CD/room)

3. Submit the CD to person in charge for each room at 7.00 AM at the day of seminar (the name of the person in charge will be announced at the day of seminar)

4. Time allocation:

i. 10 minutes presentation

ii. 25-30 minutes discussion with examiners

5. Be on time, no excuses will be accepted

6. Log book should be signed by the supervisors after the seminar

7. No food/present allowed to be brought into seminar rooms

IV. REVISED VERSION SUBMISSION

1. Submitted to SBP

i. Yellow hard cover

ii. One hard copy and CD (with file of slide presentation)

2. Dead line: June, 25^th 2010

plan well cuz those who fail to plan is planning to fail.n of coz loads n loads of luckz!n stay free of stress k.myb just some but yeah,do get loose once a while.XD

a few reminders regarding minor thesis registration:

1. go to www.fkunpad.net ONLY at FK (computer lab or laptop).
2. login.choose SATPT.choose AJUAN SKRIPSI.n u'll see this.
3.fill in JUDUL SKRIPSI by typing in the topics.
4.3 judul skripsi from 3 different department.
5.if u have consulted with any doctors,choose their name in the PEMBIMBING section.if u havent leave it empty.n if their names arent in the list,do this:

judul skripsi : nama dokter
eg. Efek obat tradisional pd mencit jantan : Dr. XYZ

6.leave other segment.
7.click save.
8.a popup window will confirm that all information is saved.
9.to check,click ADUAN SKRIPSI.
10.n u'll see all 3 keyed in topics on screen.

-tarikh tutup registrasi setakat ini adalah 6 oktober 2009 pd jam 23.59.
-today,only 10-20 computers are allowed to be used for registration purposes.if more are used,sgtlah lmbt utk loading n u cant even masuk website FK.so,if tomorrow ada announcement saying only what number of computers can be used,plz comply k.sbb nk mengisi d detail sgtlah sekejap.huu~
-xsure esok system cmna.kalo just a few laptops are allowed to be used,maybe kena antri sgtlh pnjg.hopefully they have figured out an effective system.
